    A 48-year-old woman with a 15-year history of gastroesophageal reflux disease (GERD) undergoes upper endoscopy for persistent dysphagia. The endoscopy reveals a salmon-pink patch in the distal esophagus just above the gastroesophageal junction. Biopsy from this area shows columnar epithelium with intestinal-type glandular structures and scattered goblet cells replacing the normal stratified squamous epithelium. The patient is found to have Barrett's esophagus. On follow-up endoscopy 2 years later, biopsy from the same region shows crowded, hyperchromatic nuclei with increased mitotic activity and loss of normal cellular maturation.
