Quick Answer
INI-CET follows AIIMS testing philosophy: longer multi-step clinical vignettes, 18-25 image MCQs, deeper pharmacology mechanism, and 10-20 recent-advances questions. NEET PG under NBEMS leans toward broader high-yield recall. The marking scheme (INI-CET +1/-1/3 vs NEET PG +4/-1) also makes INI-CET friendlier to calibrated guesses. If you can eliminate even one option on INI-CET, attempt it.
Why This Matters for NEET PG & INI-CET
INI-CET 2026 is on 1 November 2026. By that date more than 90,000 candidates will fight for roughly 1,100 seats across AIIMS, PGIMER, JIPMER, NIMHANS and SCTIMST. The gap between a seat at AIIMS New Delhi and a seat at a tier-3 institute is a difference of fewer than 15 marks on a 200-mark paper. That means every unit of exam-specific preparation, done in the right direction, is directly rank-moving.
The common mistake is to treat INI-CET as a smaller, scarier NEET PG. It is not. The two exams are conducted by different bodies (AIIMS Delhi for INI-CET, NBEMS for NEET PG), they test different cognitive layers, they weight different subjects, and the marking schemes imply different attempt strategies. A top-100 NEET PG rank-holder can finish outside the top 2,000 on INI-CET if they never recalibrated for AIIMS pattern. This article decodes those differences with worked examples so you can shift your approach in the final weeks.
The second reason this matters is that INI-CET quality preparation pays dividends on NEET PG. Working at AIIMS depth makes NBEMS stems feel easier, and the recent-advances layer you build for INI-CET will show up on NEET PG 2027 (whose expected date is 30 August 2027 and whose pattern now matches 180 Q / 720 marks / 5 locked sections / 210 min). You are not sacrificing one for the other — you are building a sharper floor.
Exam Pattern Side-by-Side
| Parameter | INI-CET | NEET PG (2026 revised) |
|---|
| Conducting body | AIIMS New Delhi | NBEMS |
| Total questions | 200 | 180 |
| Max marks | 200 (MD/MS), 200 (MDS at -1/5) | 720 |
| Marking | +1 right, -1/3 wrong (MD/MS); -1/5 (MDS) | +4 right, -1 wrong |
| Duration | 180 min | 210 min |
| Sections | 4 blocks of 50 Q | 5 locked blocks of 36 Q |
| Time per Q | 54 sec | 70 sec |
| Section locking | At the end of each 45 min block | Each 42 min section locks permanently |
| Image question load | 18-25 per paper | 8-12 per paper |
| Recent-advances load | 10-20 per paper | 4-8 per paper |
| Seats | ~1,100 across 5 INIs | ~70,000 across state and central quotas |
| Guess break-even | 1 in 4 (MD/MS) | 1 in 5 |
The structural implication is clear. INI-CET rewards faster readers who can trust well-reasoned guesses. NEET PG rewards disciplined attempters who leave what they do not know.
The Five Pillars of AIIMS Pattern
1. Multi-step clinical reasoning
AIIMS examiners build stems that force synthesis. A typical INI-CET medicine stem gives you an age-sex vignette, a presenting complaint, two signs, two investigations and asks for the next best step. You cannot answer by recognising a buzzword; you must integrate.
2. Image-rich testing
Visuals are a core AIIMS tradition going back to the pre-NEET-PG era. Expect pathology slides, radiology, dermatology, instruments, surgical specimens, ECG strips, slit-lamp and fundus photos. Image-only recognition is tested, but increasingly images are paired with a clinical stem so you must identify the lesion and apply its management.
3. Mechanism depth in pharmacology and physiology
Where NEET PG asks "first-line drug for X," INI-CET asks "which receptor subtype mediates the therapeutic effect" or "which enzyme step is inhibited by Y." This is a direct echo of AIIMS undergraduate teaching, where mechanism precedes memorisation.
4. Recent advances
INI-CET consistently includes drugs, trials, classifications and guidelines updated within 24 months. BPaL regimen for TB, finerenone in CKD-DKD, inclisiran, bempedoic acid, newer WHO CNS tumor classification, revised sepsis definitions, newer stroke thrombolysis windows, updated hypertension cutoffs — all have appeared.
5. Trap distractors
AIIMS loves the plausible-but-wrong distractor. The "almost right" option tests whether you fully understand the concept or are relying on a half-memory. If an option looks obviously correct within 10 seconds, re-read the stem — you have likely missed a word.
Worked Example 1: Clinical Reasoning Depth
NEET PG-style stem
A 55-year-old man with uncontrolled hypertension on amlodipine presents with pedal edema. What is the next best step?
A. Add thiazide
B. Switch to ACE inhibitor
C. Add furosemide
D. Reduce amlodipine dose
Answer: D. Pedal edema is a dose-related calcium channel blocker side effect. Classic single-concept recall.
INI-CET-style stem
A 55-year-old man, hypertensive for 6 years on amlodipine 10 mg, now reports progressive bilateral pedal edema for 3 weeks. BP today is 148/92. Urine routine shows no proteinuria. Serum creatinine 0.9 mg/dL. Echocardiography shows normal LV function. Which of the following is the most appropriate next step?
A. Switch amlodipine to a non-dihydropyridine calcium channel blocker
B. Add low-dose chlorthalidone 12.5 mg
C. Reduce amlodipine to 5 mg and add telmisartan 40 mg
D. Discontinue amlodipine and start labetalol
Answer: C. The stem explicitly rules out proteinuria (not nephropathy), rules out heart failure (normal echo), and tells you BP is still above target — so you cannot simply reduce amlodipine without adding something, and the ARB tackles the pedal edema (dose-related DHP CCB effect) by letting you use a lower CCB dose while still controlling BP. Adding chlorthalidone alone would add diuretic edema risk without the venodilator benefit.
Notice how the INI-CET stem required you to interpret three negatives (no proteinuria, no HF, normal creatinine) before choosing among two superficially similar options. This is the AIIMS fingerprint.
Worked Example 2: Image-Based MCQ
INI-CET-style image stem
A 45-year-old woman presents with a slow-growing mass in the right lobe of thyroid. FNAC shows cells arranged in follicles with ground-glass nuclei, nuclear grooves and intranuclear pseudoinclusions. Which of the following molecular alterations is most frequently associated with this lesion?
A. RAS mutation
B. BRAF V600E mutation
C. PAX8-PPARG fusion
D. RET-PTC rearrangement in sporadic form only
Answer: B. The histology describes papillary thyroid carcinoma (Orphan Annie nuclei, grooves, pseudoinclusions). BRAF V600E is the single most common molecular driver, found in roughly 60 percent of classic PTC. RET-PTC rearrangements occur but are more associated with radiation-induced and pediatric cases. PAX8-PPARG is follicular carcinoma. RAS is seen in follicular variant PTC and follicular neoplasms.
NEET PG would likely ask "which thyroid cancer shows Orphan Annie nuclei" — a one-step image identification. INI-CET asks the molecular layer on top. Both skills must be trained.
Worked Example 3: Pharmacology Mechanism Depth
NEET PG-style stem
Which of the following is the first-line antidote for organophosphate poisoning?
A. Pralidoxime
B. Atropine
C. Physostigmine
D. Neostigmine
Answer: B. Atropine is first; pralidoxime is given alongside to reactivate aged acetylcholinesterase.
INI-CET-style stem
Which of the following best explains why pralidoxime loses efficacy if given more than 24 to 48 hours after organophosphate exposure?
A. Pralidoxime is rapidly cleared by renal excretion
B. Acetylcholinesterase undergoes aging through dealkylation of the phosphorylated serine, making the bond resistant to oxime reactivation
C. Pralidoxime cannot cross the blood-brain barrier beyond 48 hours
D. Acetylcholine stores are depleted making oxime action irrelevant
Answer: B. Aging is the dealkylation of the phosphoryl-enzyme bond, making it covalent and irreversible. This is a mechanism-level question — the exam is testing whether you understand WHY, not just WHAT.
Worked Example 4: Recent-Advances Flavor
INI-CET-style stem
Which of the following combinations represents the WHO 2022-endorsed 6-month regimen for rifampicin-resistant and pre-XDR tuberculosis in adults?
A. Bedaquiline + Linezolid + Pretomanid + Moxifloxacin (BPaLM)
B. Bedaquiline + Clofazimine + Linezolid for 18-20 months
C. Delamanid + Levofloxacin + Pyrazinamide + Ethambutol for 9 months
D. Rifampicin + Isoniazid + Pyrazinamide + Ethambutol intensive phase extended
Answer: A. BPaLM is the WHO 2022 all-oral 6-month regimen for MDR-TB and pre-XDR-TB, replacing the earlier 18-20 month regimen. This is pure recent-advances recall — unlikely to appear on NEET PG unless the question is explicitly a current-guidelines item.
Marking Scheme and Attempt Strategy
INI-CET math
- +1 right, -1/3 wrong (MD/MS). Guess break-even = correct 1 in 4 attempts.
- MDS has -1/5 (break-even 1 in 6, essentially always attempt).
NEET PG math
- +4 right, -1 wrong. Guess break-even = correct 1 in 5 attempts (20 percent confidence).
Practical attempt rules
| Scenario | NEET PG | INI-CET |
|---|
| Know the answer | Attempt | Attempt |
| Can eliminate 3 of 4 options | Attempt | Attempt |
| Can eliminate 2 of 4 options | Attempt | Always attempt |
| Can eliminate 1 of 4 options | Only if you have a gut lean | Attempt |
| Blind guess | Skip | Still net-neutral; mark and move |
| Running out of time, last 5 min | Mark best gut, move | Mark best gut, move |
The upshot: on INI-CET you should finish all 200. Blanks are more expensive than calibrated guesses. On NEET PG, 15 to 25 skips is still acceptable if those were low-confidence items.
Section-Wise Strategy for INI-CET
INI-CET has four blocks of 50 questions in 45 minutes each. The order of blocks is fixed and does not let you revisit a previous block. Suggested approach:
- Block 1 (first 45 min): usually medicine, pediatrics, OBG mix. Start steady; do not rush. Mark anything that will take over 90 seconds.
- Block 2: often surgery, orthopedics, radiology, anesthesia. Image recognition heavy. Train pattern-match speed.
- Block 3: pathology, pharmacology, microbiology, biochemistry. Mechanism depth kicks in. Protect time; this is where traps live.
- Block 4: PSM, FMT, dermatology, psychiatry, ophthalmology, ENT. Recent-advances cluster. Review flagged items first, then marathon through.
Subject Weightage Comparison
| Subject | INI-CET (approx) | NEET PG (approx) |
|---|
| Medicine | 25-30 | 25-28 |
| Surgery | 20-25 | 22-26 |
| OBG | 15-20 | 15-18 |
| Pediatrics | 12-16 | 10-14 |
| Pathology | 12-18 | 10-14 |
| Pharmacology | 10-15 | 9-13 |
| Microbiology | 8-12 | 8-10 |
| PSM | 10-15 | 20-25 |
| Anatomy | 6-10 | 6-8 |
| Physiology | 6-10 | 6-8 |
| Biochemistry | 5-8 | 5-7 |
| Radiology | 5-8 | 3-5 |
| Minor specialties (Ophthal, ENT, Ortho, Derm, Psych, FMT, Anesth) | 25-35 combined | 20-28 combined |
Notice PSM is almost half-weight on INI-CET versus NEET PG. For INI-CET-focused prep, de-prioritise PSM breadth and focus on classic AIIMS-favorite PSM topics (biostatistics, epidemiology measures, screening tests, demography, immunization schedule).
Preparation Framework (Last 30 Days)
| Day block | Primary focus | Image load | Mock cadence |
|---|
| D-30 to D-21 | Deep subject revision — 2 subjects per week, Tier-1 first | 30 images/day | AIIMS/INI-CET PYQ paper every 3rd day |
| D-20 to D-11 | Recent advances + image intensives + weakest subject | 50 images/day | Alternate full-length and sectional |
| D-10 to D-3 | Rapid revision + grand tests | 60 images/day | Full-length every 2nd day |
| D-2 to D-1 | Zero new content. Flashcards, image flipping, formulas | 30 images/day | One short sectional only |
| D-0 (exam day) | No study. Rest, hydrate, nutritious meals | 0 | 0 |
High-Yield One-Liners
- INI-CET marking is +1/-1/3 for MD/MS; MDS seats use -1/5.
- Guess break-even on INI-CET MD/MS is 1 in 4; eliminate one option and you are already net-positive.
- NEET PG at +4/-1 has a 1 in 5 break-even; be more selective about blind guesses.
- INI-CET has 18-25 image MCQs; NEET PG has 8-12.
- INI-CET recent-advances load is 10-20 questions versus 4-8 on NEET PG.
- INI-CET pharmacology tests receptor subtype and enzyme step depth; NEET PG tests first-line drug and side effect.
- INI-CET PSM weightage is almost half of NEET PG PSM weightage; prioritise accordingly.
- INI-CET radiology weight is higher than NEET PG; drill X-ray, CT, MRI landmarks and classical signs.
- BPaL and BPaLM regimens are the 2022 WHO MDR-TB regimens; test-ready facts.
- Finerenone is the non-steroidal MRA with cardiorenal benefit in CKD-DKD.
- Inclisiran is a siRNA against PCSK9, dosed every 6 months.
- WHO 2021 CNS tumor classification is now molecular-driven; IDH, 1p/19q, H3K27M, MYCN all testable.
- INI-CET section locks every 45 minutes; you cannot return to an earlier block.
- Average read-and-answer time is 54 seconds per INI-CET question; train at that pace.
- AIIMS-pattern distractors are "almost right"; read the stem twice before answering fast.
Common Mistakes
- Treating INI-CET as a smaller NEET PG. The pattern, depth and timing all differ. Candidates who do not shift strategy consistently underperform their NEET PG rank on INI-CET.
- Over-attempting on NEET PG with INI-CET psychology. The +4/-1 scheme punishes aggressive guesses. Leaving 15-25 low-confidence items on NEET PG is normal; the same discipline on INI-CET costs you rank.
- Skipping recent-advances for INI-CET. Those 10-20 marks are concentrated in the current year's guideline and drug updates. Candidates who use only year-old textbooks lose this entire pool.
- Under-investing in image training. Image MCQs are the single highest-leverage block for INI-CET. One hour daily for 30 days converts most unknowns to easy marks.
- Running out of time on INI-CET block 4. Minor specialities cluster late. Candidates who over-invest in blocks 1-2 cannot finish block 4 and leave recent-advances marks on the table.
